How does it work?
If you are eligible for a Home Care Package, the Australian Government will provide you with a subsidy. This subsidy allows you to select a provider of your choice to deliver services and provide Case Management to meet your individual needs.
When applying for the funding, the My Aged Care team will assess what your needs are and how much support you need. There are four levels of Home Care Packages – the assessment will decide which level Home Care Package you need. Summa Care provides care and support on all four levels.
Basic needs - Cleaning, gardening, transport, social support or help with meal preparation.
Low needs - The same support as level 1, but with a nurse to visit and assist with medication, wound management and wound care.
Intermediate needs - This may include middle-level healthcare, such as catheter or continence management, and assistance with mobility equipment such as walking frames and bed rails. Support from level 1 and 2 is included as well.
High - Level 4 includes high-level care such as 24-hour support from medical staff, assistance with feeding, enteral feeding formula and equipment or allied referrals.
How do I apply for a Home Care Package?
- An older person living in Australia who needs coordinated support to help you stay in your home.
This form of government support is meant for older people, but there is no minimum age requirement. There are no citizenship or residency restrictions, however the subsidies are not meant for visitors to Australia or people who only need temporary/short-term care.
In order to get a Home Care Package, you or your family member needs to be assessed by the My Aged Care team and the Aged Care Assessment Team (ACAT).

2. Get your needs assessed at a face-to-face meeting
Once you have submitted your application, My Aged Care will contact you to arrange a face-to-face meeting at your home. At this assessment, your exact needs will be determined.
3. The Aged Care team will inform you what services you are eligible for
After the assessment, the Aged Care team will let you know what services you are eligible for.

What if I can't get a Home Care Package?
If you do not have a Home Care Package or are ineligible, Summa Care offers the option of Fee For Service supports, allowing you to receive the (extra) supports you would like for a fee.
This service can also be used while you are waiting for your Home Care Package to be approved, or if you would like to receive some extra services outside of your plan. So there are a few situations in which Fee For Service could be a suitable option for you:
- You do not have a Home Care Package
- You have applied for a Home Care Package and are waiting for approval, but need supports urgently while you wait
- You would like to receive extra supports outside of your Home Care Package.
